The shift to value-based reimbursement requires hospitals and health systems to take a more thoughtful approach to managing population health. This requires the development of robust analytical strategies to track quality of care and assist patients in managing their chronic diseases.
To achieve this, healthcare provider organizations need an enterprise-wide focus on integrating critical data into clinical workflows to identify high-risk patients and, importantly, provide proactive interventions. approach should be adopted. But of course, uncovering the right data for the right patients at the right time is easier said than done.
We recently spoke with Brian Dixon, a research scientist at the Indiana University Regenstrief Institute and interim director of the Regenstrief University Clem McDonald Center for Biomedical Informatics. He is a longtime leader in clinical data management and health policy, with a long-time focus on population health strategy.
He discussed strategies to improve patient-provider collaboration and involve patients more closely in their care planning. And how executive leadership can support analytics for value-based care and how clinicians can use EHRs and claims, patient-generated health data, social determinants, etc. to inform decision-making. We proposed how this information could be utilized.
We also looked at technology-enabled patient engagement strategies, how AI and automation can streamline clinical workflows and improve decision support, and how to prioritize the patient and provider experience.
