Pasco County, Florida– The Pasco County, Florida Department of Health (DOH-Pasco) wants to hear from you. The Community Health Needs Assessment (CHNA) survey is open to the public and DOH-Pasco invites all local residents to complete the survey.
The 2024 Community Health Needs Assessment (CHNA) collects data from Pasco County residents on nutrition, physical activity, access to care, and mental health. The survey is available in English and Spanish at bit.ly/49jcn1t or by scanning the QR code below. The survey will be accepted until April 15, 2024.
Data collected from CHNA surveys will help DOH Pasco better understand community needs and will be used to develop community health improvement plans that address the issues residents are most concerned about.
“Your voice helps us prioritize health needs in our communities. Knowing what is urgent drives awareness, action, and improvement.” said DOH-Pasco Health Officer Tran Chitakorn.
To participate in the survey, you must be at least 18 years old and a resident of Pasco County. The survey consists of 16 core questions and additional follow-up questions based on your responses. In addition, eight demographic questions are included. The survey will take approximately 10 minutes to complete.
